Insurance and Financial Responsibilities
Sweeney Eye Associates participates with most major insurance plans and will file your medical claim to the participating plan. Sweeney Eye Associates is a Medicare provider, however we do not participate with all of the Medicare Replacement plans. These plans are not filed to Medicare, nor are they paid by Medicare, so be sure to inform our office that you are no longer on Medicare so we may verify if we participate with your Medicare Replacement plan.
Patient co-pays, deductibles and non-covered service items are collected at the time of service. All surgical fees are collected prior to a scheduled surgery. We ask that you keep us informed of any changes with your insurance plan. If a claim is denied due to incorrect information or termination of benefits, you will be responsible for the charges incurred. If you have questions regarding your current medical or vision plan, or to inquire if we participate with your current insurance plan, please contact our office and ask to speak to our patient account coordinator. We will be happy to answer any questions you may have.
